At Health Moves Exercise Physiology, we value consistent and high-quality intervention. If you need to cancel an appointment it is recommended you contact us as soon as possible. If you contact Health Moves Exercise Physiology inside 2 clear business days before your appointment, we will charge a 100% cancellation fee.
Short Notice Cancellation – 2 clear business days
- This policy applies when a participant has provided less than two (2) clear business days’ notice of cancellation for a service, or if a participant does not show up for a scheduled service within a reasonable time or is not present at the agreed place within a reasonable time when the provider is traveling to deliver the support.
Travel will be charged if the provider is traveling to provider the service and the service is not canceled prior to travel commencing.
- Health Moves Exercise Physiology may choose to waive the short notice cancellation fee at their discretion.
- Where Health Moves Exercise Physiology cancels a support due to operational reasons, the service will be rescheduled or canceled at no penalty to either party.
- Where multiple cancellations or no shows occur in a 3-month period, Health Moves Exercise Physiology will initiate contact with the family and their support network to establish the supports we are providing are best suited to the family dynamics and the needs of the participant.
Our policy is to charge for travel from our office to the location of the appointment. The exception to this rule is we do not charge for travel to Tobruk Memorial pool for hydrotherapy services. If hydrotherapy is completed at another location travel charges will apply.
- Our travel rates are calculated using the provider’s travel time, not km’s traveled.
- Current rate is $166.99 per hour
- 60+ minutes of travel time will be charged at a maximum of 60 minutes.
Grouping community visits
When offering community visit appointments, practitioners will try, wherever possible, to group visits in the same geographical area in order to reduce travel costs. Travel costs are then apportioned to each client.
Travel costs may vary
To aid with budgeting, please note that for any community visit, the maximum travel charge would be the cost of travel from our clinic to the location of the visit on the outbound journey and back to the clinic on the return journey. This is calculated using the hourly rate indicated on our current Schedule of Fees. Naturally, the further the visit is from our clinic, the higher the travel costs may be but we will do our best to group community visits, as indicated above, and reduce this cost for you.
We only calculate travel as per our current schedule of fees, we do not add KM surcharge on top of this.
As an Australian-based organisation, any data and information collected is held, used and disclosed in accordance with the Privacy Act 1988.
‘Personal health information’ is a particular subset of personal information and can include any information collected about a person in order to provide a health service.
The information we collect about a patient can include medical details, family information, name, address, employment and other demographic data, past medical and social history, current health issues and future medical care, Medicare number, accounts details, and any health information such as a medical or personal opinion about a person’s health, disability or health status.
Personal health information also includes the formal health record (written or electronic) and information held or recorded on any other medium (e.g. letter, facsimile, electronic, verbal).
Our security policies and procedures regarding the confidentiality of patient health records and other personal information are documented and our practice team are informed about these at induction and when updates or changes occur.
For each patient we have an individual patient health record containing all clinical information held by our practice relating to that patient. Our practice ensures the protection of all information contained within these files. Our patient health records are accessed only by an appropriate team member as required, and we ensure information held about the patient in different records (e.g. at a residential aged care facility) is available when required.
The Privacy Act 1988 and the Australian Privacy Principles require our practice to have a document that clearly sets out its policies on handling personal information, including health information.
This document, called a Privacy Policy, outlines how we handle personal information collected (including health information) and how we protect this information.
Patient consent to the handling and sharing of personal patient health information is sought and documented early in the process of clinical care, and patients are made aware of the collection statement when giving consent to share health information.
According to the Privacy Act 1988 and the Australian Privacy Principles, an organisation may use or disclose personal health information for a purpose (the secondary purpose) which is directly related to the primary purpose of collection without seeking consent, but only if the individual would have a reasonable expectation that the information could be used or disclosed for that secondary purpose.
A directly related secondary purpose for the use and disclosure of personal health information in our practice includes the many activities necessary for the provision of a health service, such as management, funding and monitoring, as well as complaint-handling, planning activities.
It is essential to recognise the importance of ‘reasonable expectation’ as many individuals may be unaware of the range of activities for which their personal health information may be used and disclosed, such as the accreditation processes. Our practice ensures we tell patients how, and for what purpose, personal health information collected about them could be used or disclosed. Patients are advised of this ‘secondary purpose’ in a number of ways, including:
At the time of the consultation with a practitioner
By reading, understanding and signing a new patient information form when first registering at the practice, which incorporates the practice privacy statement.
It is important we maintain a patient’s right to ‘opt out’ of the secondary purpose through refusal to consent. If an individual expresses negative views or opposition when made aware of a proposed secondary use or disclosure of their personal health information, this would indicate that they have a reasonable expectation that their personal health information will not be used or disclosed in that manner, and their non-consent is recorded on file.
Complaints are made when a client, advocate, entity or member of the public experiences dissatisfaction with an organisation or an outcome they have received.
Opportunities are available for patients and other visitors to tell us, “How we are doing”.
Individuals have a ‘right to complain’ and where possible, stakeholders of Health Moves and others are encouraged to raise any concerns directly with the practice team who are all trained to make sure patients of the practice feel confident that any feedback or complaints made will be handled appropriately. We believe most complaints can be responded to and resolved at the time the patients or other people such as carers, relatives, friends, or other consumers make them known to our team.
All members of our practice team are educated to be prepared to address complaints as they arise. Depending on the nature of the complaint and any advice received from our medical indemnity insurers where required, complaints are recorded and actioned with a copy placed in the patient’s health record if related to patient care.
All clinical staff, and administration staff, are aware of the professional and legal obligations regarding the mandatory reporting of unprofessional conduct.
If an Individual wishes they can take their complaint directly to the NDIS Quality and Safe Guard Commission.
– The National Quality and Safeguards Commission encourages individuals, in the first instance, to contact your provider if you feel comfortable doing so – this is the best way to resolve your complaint quickly.
– However, if they feel the complaint has not been resolved to your satisfaction, individuals can seek further support from an independent advocate – further details can be found on the NDIS Commission website, or you can contact the NDIS Commission directly. Further information can be found at:
